A few things to know about LMNs:
- Your healthcare provider, not TheraBulb, determines whether an LMN is appropriate for you.
- Many administrators expect the LMN to be dated on or before your purchase date, so it is wise to obtain one first.
- LMNs are typically valid for 12 months and should be kept on file with your receipt for at least three years in case your plan or the IRS requests documentation.
How Reimbursement Typically Works
Step 1: Check Your Plan
Contact your HSA administrator (or review your program guidelines) to confirm whether TheraBulb products qualify under your plan and whether an LMN is required.
Step 2: Obtain Any Required Documentation
If your plan requires an LMN, speak with your healthcare provider before making your purchase.
Step 3: Make Your Purchase and Save Your Records
Your TheraBulb order confirmation serves as an itemized receipt showing the merchant, purchase date, product name, and amount paid.
Step 4: Submit Your Claim
File your reimbursement claim with your plan administrator, including your itemized receipt and LMN if applicable.
Step 5: Retain Your Records
Save your receipt and supporting documentation for at least three years.
